ssion, substance use disorders, and chronic health conditions than those with no ACEs. This score warrants comprehensive trauma-informed assessment and intervention. Analysis of the ACE-Q dimensions shows a score of 5 out of 5 in the Abuse/Neglect domain and cumulative risk and may require comprehensive intervention addressing both individual trauma and systemic factors. The client reported the following adverse childhood experiences: - 1. A parent or other adult in the household often swore at you, insulted you, put you down, or humiliated you, or made you afraid that you might be physically hurt. - 2. A parent or other adult in the household often pushed, grabbed, slapped, or threw something at you, or ever hit you so hard that you had marks or were injured - 3. An adult or person at least 5 years older than you touched or fondled you or had you touch their body in a sexual way or tried to or actually had oral, anal, or vaginal sex with you. - 4. You often felt that no one in your family loved you or thought you were important or special, or your family didn't look out for each other, feel close to each other, or support each other. - 5. You didn't have enough to eat, you had to wear dirty clothes, and you had no one to protect you, or your parents were too drunk or high to take care of you or take you to the doctor if you needed it. - 6. Your parents were separated or divorced. - 7. Your mother or stepmother was often pushed, grabbed, slapped or had something thrown at her or was sometimes or often kicked, bitten, hit with a fist or hit with something hard, or was repeatedly hit over at least a few minutes or threatened with a gun or knife. - 8. A household member was a problem drinker or alcoholic, or used street drugs. - 9. A household member was depressed or mentally ill or a household member attempted suicide. - 10. A household member went to prison. Sexual Abuse Note: The client has endorsed experiencing sexual abuse during childhood (Question 3). Research by Briggs et al. (2021) identifies sexual abuse as the most reactive adverse childhood experience, indicating that it can interact with other adverse experiences to create disproportionate impacts on health and wellbeing. Even when the total ACE score falls elow the traditional clinical threshold of 4, the presence of sexual abuse warrants significant clinical attention, particularly when combined with other adversities such a: neglect or physical abuse. In these combinations, synergistic effects may account for up to 0% of the variance in negative outcomes regardless of the total ACE score. This suggests eed for particular clinical attention to this experience and its potential contributions to curren presentations. A trauma-informed approach that specifically addresses sexual trauma may be indicated. Treatment Considerations: • Context matters: ACE scores should be interpreted within the context of the individual's current functioning, symptom presentation, and available protective factors. • Consider that individuals with higher ACE scores often require longer courses of treatment and may benefit from approaches that specifically target affect regulation, attachment disruptions, and maladaptive beliefs stemming from early adversity. • Psychoeducation about the impact of early adversity on current functioning can help normalise sympto
